What should I take to my medical assessment?

There are a few things to remember to take with you to your medical assessment appointment. Failure to do may mean that you cannot complete part of your assessment.

  • Ensure you have at least one valid form of Photo Identification (eg Drivers License, Passport etc)
  • Take any visual aids (glasses, contact lenses) that you require.
  • If you require any hearing aids, bring them with you.
  • It can be helpful to bring any records outlining on-going treatment for any medical conditions you may be managing (for example high blood pressure, diabetes etc).
  • If you are having a travel medical assessment or require vaccinations, please bring any vaccination records with you.

Gemini Medical will provide the correct paperwork (according to your prospective employer's instructions) to the clinic, so you should not have to worry about bringing paperwork with you. If you are required to bring your own paperwork, you will be advised to do so by either your propsective employer or by Gemini Medical.

If you have been advised by your prospective employer that you are responsible for paying for your medical assessment, you will need to ensure that payment is settled with the clinic on the day you attend your appointment. Failure to do so can delay the release of results from the clinic to Gemini Medical.
